u need not open a savings account seperately.
Demat refers to a dematerialised account.
Just as you have to open an account with a bank if you want to save your money, make cheque payments etc,

No. But you do need to have a bank account (savings or checking). It does not matter whether it is at ICICI or another bank.

You need to have a brokerage and DMAT account with ICICI for sure if you want to do trading with them. They may also insist you to open bank account so you have 3-in-one (bank, trading and DMAT) in on

Yes.
KYC is being strictly enforced by RBI / SEBI therefore these days all banks enforce that you have a SB account with them.
But the story is different if you want to have a DMAT with non-bankin
